All Hours Adult Care

receive care in an assisted living setting instead of a nursing home.
Through All Hours Adult Care, this program helps individuals find and access assisted
living options that provide support with daily needs such as bathing, meals, medication
reminders, and supervision.

Helps coordinate care needs, referrals, and support services so eligible individuals can access the right placement and support.
Assisted living placement support
Eligible individuals can receive support in a residential care setting instead of a nursing home.
